  • How bad the pain is on a scale of 0 to 10 (zero being no pain and 10 being the worst you can imagine)?
  • Where it is (diagrams can help with this).
  • How long did it last?
  • What other symptoms it was associated with. For example, bleeding or nausea.
  • Things that aggravate the pain. For example, do certain activities make it worst? What about when you are stressed and/or tired? Or if you've had an argument with your partner?
  • Things that make the pain less or more manageable. For example, pain relief, exercise, lying a certain way, or a good night's sleep.
  • What does the pain feel like? For example, a sharp stabbing, a dull ache, or a dragging sensation (common with endometriosis).
  • How does it impact your life?
